Eligibility

To be eligible for HCS or TXHML services at N2 Care Services, you should have the following:

• A qualified documented diagnosis of intellectual disability or related condition as
• stipulated by state laws.
• Medicaid eligibility.
• Enrolled in HCS or TXHML program with an authorized Individual Plan of Care (IPC),
• Level of Need (LON) and Level of Care (LOC).

1. On the HCS/TXHML waiting list.
2. Currently enrolled in the HCS/TXHML program but dissatisfied with your current provider.
